This is not a ‘what do you think of your GP practice’ survey and certainly not an opportunity to criticise GP practices.
We cannot stress enough that the point of this survey is to understand what is needed in the system to drive change and direct resources to meet your needs and the needs of our communities, namely those with a chronic condition.
Without your input we cannot get the whole picture.
