This survey is important as it will give us evidence of the current pressures faced by GP practices and any un-met needs of our communities.
It is the first study of its kind into people living with chronic conditions and as a result that means for the first time it will allow us to see what is needed in the system.
We understand the pressure GP practices are under, but this data provides us with an evidence-based current state of play. It will help policy makers better understand how, and where, health systems can be improved. That is why we are asking you to take part.
