To ensure the survey is representative, we have carried out sampling on all the GP practices in Wales, based on factors such as how many patients each practice has, the area covered by the practice etc.
Your practice has been identified through this proportionate sampling strategy, which is why we are asking you to take part.
It also means by completing we will be able to invite a sample of your over 45 patients to take part.
