What will you ask our patients?

Patients will be asked about their general health and quality of life.

Some examples of the questions they will be asked are:

  • To what extent are you able to carry out your everyday physical activities such as walking, climbing stairs, carrying shopping, or moving a chair?
  • Is there one single professional you usually go to for most of your health problems?
  • How often do you eat fruit, excluding juice squeezed from fresh fruit or made from concentrate?
  • How many different medications as prescribed by a doctor or a nurse are you taking on a regular or ongoing basis?
  • How easy is it for you to use your GP practice’s website to look for information or access services?
  • Should you need help, how easy is it for you to get help from the following people?  A close family member (including your partner) or friends, neighbours and co-workers?
  • Have you ever smoked tobacco products (excluding electronic cigarettes or similar electronic devices) daily, or almost daily, for at least one year?

As you can see the breadth of questions is broad and you can start to see the invaluable data and insights we will receive from the final report.  Likewise, the sort of information you will be able to use in your own practice.

As mentioned throughout, these FAQs this is not a “what do you think of your GP practice” survey and will not be used to criticise GPs and GP practice staff.

If you wish to see a version of the patient questionnaire please get on touch and we will be able to share a copy with you. Please email our team on ViH@wales.nhs.uk
